Set in a rural Kerala village, Akkarappacha tells the emotional saga of Bhagirathi and her impoverished family trying to maintain dignity against overwhelming odds. Bhagirathi works relentlessly in a cashew factory to support her younger sisters, Janamma and Devaki, along with her young daughter. Meanwhile, her unemployed relative Vasu spends his time indulging in card games and lofty ambitions with local loafers rather than helping the household. Romantic complications arise when Janamma falls in love with Vijayan, a young military soldier, while local rogue Bhaskaran also casts eyes on the family's women.
Gossiping neighbors and internal family disputes over property rights create severe misunderstandings between the sisters. As Vijayan returns home on leave intending to marry, rumors and jealousy spark fierce arguments within the family, causing Bhagirathi to suffer immense emotional distress and societal humiliation. Janamma and Vijayan eventually marry, but the unresolved guilt, personal sacrifices, and physical toll take a heavy toll on Bhagirathi. In a dramatic and heart-wrenching climax, Bhagirathi collapses due to extreme hardship and emotional strain, passing away in the presence of her reunited family, leaving behind a legacy of selfless love and maternal sacrifice.
